This session focuses on the challenges and opportunities to improve care among individuals transitioning at various stages of CKD and dialysis care.

• Identify challenges in transitioning from pediatric to adult home dialysis
• Specify challenges in transitioning to and from kidney transplant and home dialysis
• Describe opportunities to improve transitions between peritoneal dialysis and home hemodialysis
• Summarize opportunities to improve coordination and care for patients transitioning from home to incenter dialysis
