Long-Term Kidney Outcomes in Pediatric Renal Vein Thrombosis

Background
Pediatric renal vein thrombosis (RVT) may increase the long-term risk for adverse renal outcomes. Most literature focuses on neonatal RVT, with low emphasis on long-term renal outcomes. There is sparse data on renal outcomes following RVT in older children. The goal of this project is to determine long-term renal outcomes of RVT across the pediatric age spectrum.
Methods
The PHIS (Pediatric Health Information System) database was used to identify patients from birth to 21 years of age with a diagnosis of RVT. We looked for the occurrence and persistence of hypertension, proteinuria, and eGFR reduction. Secondary outcomes analyzed were end stage renal disease and mortality.
Results
We identified 516 children with a diagnosis of RV in the PHIS database (Table 1). 51.9% were male. The mean age for RVT diagnosis was 2.6 years, the most common age at diagnosis was in the neonates. Children as old as 17 years of age were also identified. 20% of the affected individuals had residual CKD. Residual HTN occurred in 30.6% of the patients. (Table 1).
Conclusion
Pediatric RVT is a risk factor for the development of CKD and hypertension regardless of age at RVT diagnosis. Survivors of pediatric RVT should be routinely screened for evolving CKD and/or hypertension. We plan a deeper analysis of these data to determine if anticoagulant therapy alters long-term renal outcomes following pediatric RVT.
