Impact of Anemia on Echocardiographic Parameters in Asymptomatic Patients with CKD on Hemodialysis

Background

Anemia and cardiac abnormalities play a crucial role in the complications associated with chronic kidney disease (CKD). However, there is insufficient information regarding the impact of anemia on cardiovascular outcomes

Methods

A cross-sectional multicenter study of hemodialysis patients with CKD was conducted. Data was collected from electronic medical records, cardiac function was assessed with echocardiography. Spearman rank correlation analysis and linear regression models adjusted for potential confounding variables were performed to assess anemia and cardiovascular remodeling. A p-value < 0.05 was considered statistically significant.

Results

Among the 124 individuals included (median age: 48 years), groups were divided by hemoglobin (Hb) level in non/mild anemia (Hb >10 mg/dL) and moderate/severe anemia (Hb <9.9 mg/dL) (Table). The moderate/severe anemia group had increased left ventricular (LV) posterior wall thickness (11mm vs. 10mm), increased left atrial (LA) volume (39 mL/m2 vs. 35 mL/m2), and decreased left ventricular ejection fraction (LVEF) (55% vs. 60%). Negative correlations (Figure) were found between Hb levels and LV posterior wall thickness (r= -0.22, p=0.013) and LA volume (r= -0.18, p=0.038). Linear regression models indicated a positive association between Hb and LVEF, and negative associations between Hb and LA volume, LV posterior wall thickness, and septum thickness.

Conclusion

Anemia degree was associated with multiple echocardiographic parameters, revealing complex interactions. This study highlights the importance of further prospective research to improve early identification and stratification of anemia in CKD patients to reduce subsequent cardiac complications.
