Introduction
Hemodialysis (HD) is often the preferred modality in pregnancy as patients undergoing PD have been found to be less likely to deliver successfully, historically. There have also been concerns over difficulty achieving necessary clearance with peritoneal dialysis as a pregnancy progresses, resulting in various possible maternal and/or fetal complications.
Case Description
We present a case of a patient with end stage renal disease due to biopsy proven class IV lupus nephritis on continuous cyclic peritoneal dialysis (CCPD) who was maintained on PD throughout her pregnancy. She required careful lab monitoring and frequent prescription adjustment to maintain adequate clearance throughout her pregnancy. She went into labor spontaneously at 33 weeks and had an uncomplicated vaginal delivery.
Discussion
With close monitoring, and careful adjustment of the dialysis prescription and a patient’s willingness to adjust to the changing and intensifying prescription, PD can be safely maintained throughout a pregnancy.
