Session Description
Transitioning from pediatric care to adult nephrology presents many challenges for adolescents or young adults with kidney diseases. This session highlights the challenges facing these patients, examines the importance of non-kidney comorbidities in determining outcomes, introduces adult nephrology professionals to relevant aspects of pediatric care, and shares best practices for producing an optimal transition that supports patients and maintains a high level of care.
