Session Description

Transitioning from pediatric care to adult nephrology presents many challenges for adolescents or young adults with kidney diseases. This session highlights the challenges facing these patients, examines the importance of non-kidney comorbidities in determining outcomes, introduces adult nephrology professionals to relevant aspects of pediatric care, and shares best practices for producing an optimal transition that supports patients and maintains a high level of care.

• Describe the psychosocial and developmental challenges facing adolescents as they transition to adult providers
• List the unique considerations of pediatric kidney transplant for adult care
• Identify the non-kidney sequelae of pediatric CKD that affect lifelong health and well-being
• Summarize real-world clinical care models that support the transition from pediatric to adult nephrology
• Discuss the patient and caregiver perspective regarding the challenges facing adolescents as they transition to adult CKD care
