Ancillary Meeting Space Request Form
Renal Week 2008 - November 4 - 9

Deadline is Friday, August 8, 2008. Requests received after deadline will be subject to $200.00 processing fee.

Important Information

If you require multiple events, please complete ONE form for EACH function you would like to hold.

Once the function has been approved, you will receive written confirmation from ASN approximately one business week after your form was received (you will be notified of the room assignment after August 14). Your request will then be forwarded to the hotel and the hotel will contact you directly for your meeting requirements.

Your organization is responsible for payment of any charges for catering, audio visual, etc. You will need to provide billing information directly to the hotel.

Deadline date for this form is Friday, August 8, 2008. Ancillary meeting requests received after this date will not be published in the ASN Schedule of Events and will be subject to a $200.00 processing fee.

Organizations may NOT hold events during ASN’s defined "blackout" times. Ancillary functions may only be hosted by ASN Affiliates, Exhibitors, or organizations affiliated with ASN. All scheduled events must adhere to ASN regulations and may NOT take place during the following specified dates:

Hotel Preference
Please choose your 1st, 2nd, 3rd preferred location. Space will be assigned on a first-come, first-served basis after the ASN Committees and Specialty Section Meetings. ASN has the right to change the space assignments. If you have a meeting at an ASN hotel this is not listed below you still must complete and return ancillary form and you may be subject to room rental fees.

ASN intends to publish a schedule of all ancillary events in the onsite program book. In order to publish information regarding, your event this request form must be submitted to ASN no later than August 8, 2008. Please indicate your preference below. If you do not indicate your preference, ASN WILL NOT publish your event. Please note, requests received after August 8, 2008 will NOT be published.
